Hi, it sounds like the engine is ticking over and revving ok i think you have to look at the drive connection, clutch maybe not connecting correctly if its dying when you put into gear.

if it dies when put in gear, try giving it gas while holding the brake, them putting it in gear if it does not die then replace the idle control valve iac it located on top on the throttle body

When i put it in gear it starts grinding like it wont go all the way into geer

I assume you have a manual transmission. If so sounds like your clutch maybe shot. To test your clutch put the park brake on while the truck is stopped.. shift into 5th gear and slowly let release the cluth.. the engine should die. If the engine does not die you clutch is slipping and needs to be replaced. How many miles on the vehicle ?

Wife was driving truck up a hill when it suddenly stalled out. she finally got the truck back down the hill. Now it will not idle properly, and stalls out if put into gear( R or 1st). Is this a carburator...

Depending on youre mechanical ability you can spend 20 bucks for a rebuild kit on your carburator.

There tends to be many different potential issues with youre description of the problem. My best advice for you at this time, would be get the carburator re-built and check youre fuel pump/fuel filter.

But you can always go to youre local shop and get a free estimate.

The engine dies when i put into gear 1st time but not after.

Sounds like idle speed motor may need cleaning, or replace. Doesn't cost more than $5-$10 for a can of throttle body cleaner, and some time, to pull it off & clean & retry before spending too much on other areas. Tuned up, and fuel filter changed lately?
