Okay, I was not there when my mother purchased this Vehicle. I've actually not heard much about this particular Van. I've been researching somethings, and asking some questions in multiple places. So...
When this Van is placed into Park, and is shut-off, there is a lot of "sway" room. As in, you can physically push the Van a good 6 inches forward. I've never had this before on any Vehicle I've owned. Is it possible the transmissions parking pawl is slightly failing?
Another semi-transmission related question. When the Vehicle is running, and coming to a stop, you can hear the grinding and engage/disengaging. I'm unclear if it's the brakes, or the actual transmission. When you release the brake pedal, at say a stop light, the Van doesn't seem to engage Drive, and barely rolls forward. This is why I'm unsure of which is the problem. The transmission or the brakes. There's no pulsating in the brake pedal, but it does seem squishy, and/or kind of sticky. When you release the pedal, there's a slightly forward jerk when the accelerator is pressed, even very lightly.
I've also noticed the carpeting seems to have wrinkled and bubbled somewhat. I'm unsure if it's from heat, water, or replacement. I found some condensation in the left front headlight cover. It has not rained here within the last 2 weeks. This kind of leads me to believe the Vehicle was sitting in water for an indiscernible amount of time. Which leads me back to the brake/transmission issue. Could that cause what the Vehicle is experiencing?
I'm asking these questions Online, because I want to get a somewhat decent feel for anything before notifying my mother. She was so happy to find a Vehicle for herself. :/
First & only suggestion I have, is get rid of it, and any van type vehicle

Having once owned a trans repair shop, I will tell you they all have bad
transaxles & that is what you have
It is tens years old,the trans was done no matter what vehicle you bought
You mentioned water in headlight & more issues
That vehicle is going to cost too much to straighten out --- UNLESS
you do (100%) 100% of ALL WORK yourself & the trans is not something you can do yourself or even install one

I agree with the post about the tail lights. And the symptom about the brakes on when starting was also helpful. When I took off my rear tail lights and removed the top light bulb (top of the 4 is closest to connector) then repeat starting the car the problem is gone. I have a whole drawer of these bulbs that I removed, not because the filament was burned out, but because the base (similar to circuit card edge connector) had heat damage and melted. I am not sure exactly what the problem is but I think there are multiple conductors in the fixture which are too close or get moisture shorts. Anyway I hypothesize the circuit for daytime running lights is leaking current into the brake light circuit, and it causes that module that controls traction control and ABS to declare a fault. When you start with foot on the brakes the brake lamp is pulled on (I don't know if switches hi or lo) so that module doesn't declare the fault until you release the brakes.
I put in new bulbs and it works fine for now, but I know it may reappear some day then I will repeat, disassembly of the fixtures and inspect bulb bases for heat damage and clean out and replace. That is what did the trick for me.

One of three things. Inside the trans are clutches, bands, sprags and planetaries. I believe that either a set of clutches is sticking together or the input sparg is not free wheeling when it is suppose to or the parking pawl is sticking in place until you free it.
There is not much you can do. The only possible item that can be added that might help is called Lube Gaurd In the red bottle. other than that you will have to pull the trans and repair it.

just to let you know, I worked for GM from 90-93 rebuilding those transmissions. I have rebuilt as least 500 of them. I could rebuild them blindfolded, (No kidding, blind folded)

Follow the accumulator cable from the firewall to the intake manifold/throttle body air tube.
It will end right next to the TPS.
TPS is a 3 wires electronic unit.
