How do you replace the tubes that go to the rear heater core on a 2000 dodge grand caravan

    the tubes run down the passenger side off the van .what i can see they run from the front heater core to the back heater core.under the front passenger door they are spliced together with heater hose

Under the hood is a drain for the cowling under the wiper arms. There is a plastic tube that gets clogged and water can't drain from that area. It is against the firewall on the passenger side. I found mine full of pine needles

A call to the dealership should be able to get you the official name but I would desribe them as the rear heater core lines. These lines basically supply heated engine coolant for the rear heat. A dealership parts dept can pull up a diagram of the vehicle based on the VIN number and give you the exact name and part number.

Heater does not blow hot air , only warm

it's possible that you have an air bubble in the cooling system and it will need bled. also possible that the heater core has a partial blockage.
