A couple of other things you might want to check are your heater control valve that is controlled by a vacuum line and also make sure you do not have any vacuum leaks in the system.

Check your antifreeze level to make sure it is up where it should be. If that isn't the case it is probably your thermostat most likely. If it isn't one of these it could be your waterpump by my bet is on the thermostat. Pretty easy cheap fix too.

check the coolant level. b-4 you start vehicle in morning, open radiator cap then start vehicle if level drops add until full then replace cap.

There is a recall that has something to do with the air/heat system in 2005 Dodge Grand caravans. Recall F01. Here is a link. I had mine done two weeks ago, cost me nothing. My air conditioner was overheating.
