Re: 03 f150 heat only comes out defrost
You have a vacuum leak. Defrost is the default postion (no vacuum). Since it is working correctly in the ac postion I would suspect the problem is at the floor/panel door. This one is located on the drivers side of the heater box just to the right of the accelerator pedal.

Dustin, Sound like vacuum loss to position switch. Defrost is the default position when vacuum is lost in vent control system. Check any and all vacuum lines and connections under hood first, especially any that run through firewall. Check the attached links, instruction and guides, Good luck

Try setting the climate control to "Automatic," then raise the temperature until the display reads what you want, like 72 or 74 degrees. The heat will come on automatically and adjust by itself when the temperature you set is reached. You can also push the "Floor" or :Defrost" button and this will turn the heat or defrost on manually.
check the steering linkage of the front end for loose components, most can be checked with the vehicle on the ground. Also , check the Intermediate Steering Shaft for play, this runs between the steering gear box & the lower part of the steering column. it has an expandable section with 2 overlapping shafts that can wear & become loose with age & driving

On cars with automatic climate control: To enter diagnostic

Floor, Defrost\Floor, Front Defrost simutaneously for 3 seconds until
the temperature display begins to flash. On cars without automatic climate control, set the blower to
any speed, turn the temperature control in between warm and cold. Press

Front Defrost button.
When the test is over all fault codes will be displayed (on cars
