Both sides ? the only thing i can think of then is either an air pocket or the thermostat is installed in reverse... (it happens).

add antifreeze to the overflow tank and squeeze the upper rad hose a few times while it is hot and see if that helps

My 2004 Chrysler T&C's heater works great when I first turn it on in the morning, while driving to work it turns to only cold air. What could be causing this issue?

That's weird - it seems like the thermostat is open when you start the mini-van (its supposed to be closed at engine start-up), and then the thermostat closes as the warms up (its supposed to open).
If you can replace thermostat, that may be the thing to do, since these thermostats often cost less than $10. Once the T&C has a new thermostat, see if the heater behavior improves, if so, discard the old thermostat.

Also, if your thermostat is actually behaving the OPPOSITE of a normal thermostat, then you're at
risk of burning up (overheating) your engine - on any trip longer than the commute to work. An engine at operating temperature needs to circulate coolant to/from the radiator (and/or heater core) to avoid overheating. So thermostat replacement may save your entire engine. In the meantime, your T&C will avoid self-destruction only if driven REALLY short distances.

If my car heater doesn't throw out heat just cold air what can it be

either thermostat needs repalcing or air trapped in heater. or blend door in plenum case not working. feel heater hoses going into dash panel behind engine and see if both are hot.

Heater core has been replaced and still blows minnimal warm air.

IF YOU HAVE A HEATER CONTROL VALVE IT MAY BE PLUGGED OR STUCK HALF WAY CLOSED. THIS VALVE IF YOU HAVE ONE WILL BE ON ONE OF THE TWO HOSES GOING TO YOUR HEATER CORE. USUALLY CABLE OR VACUUM CONTROLLED. REPLACE THE HEATER CONTROL VALVE AND IT WILL RESTORE THE WATER FLOW TO THE HEATER CORE.

92 Mustang Engine WIll Not Heat

Sounds like you have partialy blocked heater core, not enough flow to the heater or heater tap either not working or only partialy open. You may have to remove heater hose at the fire wall and flush with garden hose. Also check if heater hoses are clear and running freely by using the same garden hose If you put hose up removed heater hose you should get a strong run of water coming out of the heater radiator.
