Question about 2001 Ford Windstar
I just had that problem and took in to the shop. Had the spark plugs changed and had a bad wire. Because of this I also ended with carbon build on the cylinder which they cleaned and had to have my fuel injectors cleaned because we had used fuel injector cleaner thinking that was the problem but caused another problem of gunked up injectors.

SOURCE: engine misfires
You didnt mention having changed the spark plug wires, so that is another item that could be culprit as well. As well, it could be a coil pack that is not working properly.
I cannot recall at the moment if that model year uses coil on plugs or if it uses coil packs. Either way, if it uses a coil on plug set up, take the one for the #3 cylinder and move it to say #4 or anyone of your choice and see if the misfire follows
If it uses coil packs, there is likely one coil pack that controls two cylinders. In that case just swap the coil pack with an adjacent one and see if the problem follows. Pay attention to where the spark plug wires go because it will be an issue if you get them mixed up. You just swap the coil packs around.
I would, however, start with the spark plug wires if you have not changed them already.

Ignition Coil Pack
Clog or dirty fuel injector
Evap canister purge valve
you have 3 type of engine for your truck
2.3L 4 cylinder
3.0L 6 Cylinder
4.0L 6 Cylinder
need to know that type Engine
The reason why the service light come on because it is a hard code
not a soft code.
Let me know

P0401 shows an Exhaust Gas Recirculation Flow Insufficient. Without a good scan-tool you will be playing the "Parts Changing Game". Insufficient flow can be the result of a vacuum leak, bad MAP sensor, an exhaust leak, faulty EBP sensor, carbon build-up, and so on. I wish I could say more but with all the electronics it is more complicated then earlier cars.
