Raditor full but when throttled up sucks down water at idle full again is that normal

I think it is caused by the water pump pushing water round the system.
As long as there is no loss then all should be fine.
I had a problem once with a leakie radiator cap only under pressure when driving. Fixed with a new radiator cap

Vacuum leak from brake booster. If you hear a sucking noise when you press the brake pedal replace it. Another way to check it is to place truck in gear set e-brake press brake pedal repeatedly and see if idle goes up and down. DO NOT PRESS GAS PEDAL DURING TEST.

Try to block all vacuum pipepes and see if the idling comes down there if its still high take a container full of water like a 2ltr the n start the car and pour the water note throttle body you may have throttling bushes worn out they will suck in the water and temporary reduce idling thats how you diagnose the vacuum leakage

My rad cap is not ho and i have heat when sitting and when driven i have no heat

your raditor is not full.
leave cap off and run motor at idle till warm then look at water level.
when the thermastat opens the water lever will drop then add and watch again till it stays at top.
when water is low it will not put out much heat.

You either have the thermostat in upside down or you have an air pocket and the system needs to "burp". Sometimes this can be done by warming the engine and letting it cool down.

Two things it might be the cars raditor fan doesn't work, or the therostate is sticking. ( the gauge does'nt have to be right) the other thing might it need a new raditor cap. Most all cars pust raditor fluild to the overflow when thier shut off if it's low it'll be gas not coolant. the hose between the cap and the tank may have a small leak and it's not sucking the coolant out of the resivoir back to the raditor. The system is designed to be completly full(no air). If you can remove the cap with the car cool and it's not full clear to overflowing the cap or the hose is bad. Being it's nor leaking I'd go with the cap. Get the kind that came with the car not one with a lever.

Try spraying the idle air passage in the throttle body with a carbuator cleaner while the engine is running, you will have to hold the throttle to keep the engine from dieing.If that helps a little than you may have to remove the idle control valve from the throttle body and try cleaning it out with a carburator spray cleaner and also spray clean the idle passge in the throttle body.Or you may have to replace it,
The idle vale should be on the left side on the side of the throttle body -- passenger side --- with two torx head screws holding it down. It has a three wire quick connect plug that looks like a race-track--- rectangular/oval.

Water in intake...not much

sounds like a head gasket is leaking, or worse, one of the water channels is cracked in the head.

a teardown is your best bet... sorry for the bad luck

next time it happens try spraying water in mist form around the throttle body if it bogs, replace the throttle body gasket. if it doesn't bog you may have a bad wire wait until dark start the vehicle have someone feed the fuel very slowly and watch for arching from a wire it will be easy to see when it is dark, replace that wire.

High idle

Your truck should have throttle body injection. Most of the electronic issues that will cause a high idle will turn on your check engine light. If it's on read the codes and check it accordingly. If it's not on then you most likely have a vaccum leak. The most common by far is a failed base gasket for the throttle body. Second is the hose for the PCV valve failing and splitting. You can check for these leaks by using some water in a spray bottle. Spray the water at the suspected leak while the engine is idling. If it's leaking you will hear the water getting sucked in or the engine note will change.
