Every time it rains my serpentine belt comes off on my 1999 plymouth grand voyager
Verify that you have a good/correct belt then check your belt routing and alignment. Then check pulleys for faulty bearings. A free spin with the belt removed should be enough. Whats really common on those is the belt tensioner goes out

try autozone.com to see if they have your car listed for diagrams pictures and step by step instructions if your car is not listed then try your local library they have all the car manuals and even copy them for you

remove belt and check the pulleys. this belt routing is odd and its very tight. i have seen many failed tensoner pulleys on these. it should spin smoothly and quiet. and grinding or bumpyness in it and i would replace it. check all pulleys while your there. they are not expensive.

Yes, the belt tensioner can wear out. I had the same problem with a brand new belt and noticed that the belt tensioner was "moving" alot when the engine was running and the belt was on (of course). I changed the belt tensioner and put the belt back on. Knock on wood, hasn't given me anymore trouble.
