Question about Cars & Trucks
If your RPMs are very low (engine almost stalls) then after 30 seconds or so goes back to normal....you might have a bad Intake Manifold Gasket....If it is a noise (rattle type) might be the Valves or Valve Springs....do to the oil not being there....there are a lot of other questions that need to be answered before one can make an educated guess on this subject; more information needed please.

you can try a tune up first or you can have your compression check you can have a crack head..it will keep an air bubbles in the water an mess with the temperature sensor. but you must be adding water to the radiator. do you have any water in your oil? if not a major tune up will fix you up.

Start with fresh oil change and filter. Make sure you are using the correct manufacture recommended oil wt. and rating. The counter person at the part show should know what to give you.

SOURCE: Engine noise cold
these motors made some noise when cold normally due to the clearance. Is it more than that? Have you tried removing plugs and soaking pistons overnight with topengine cleaner this will loosen carbon which makes some cold noise problems.

Timing chains don't require replacing like a belt,
unless you got some not so good problems
