Remove the metal piece that glues onto window, probably with an allen wrench. It is a set screw. Clean the metal and the window where it attaches of old glue, then wipe with rubbing alcohol. Use the cement sparingly, and let the metal piece set up a few hours or however long the cement requires, before attaching the mirror back on.
Be vigilant, I put one on upside down, once.

Go to any auto supply store and request rear view mirror cement and reapply the mirror with it.

SOURCE: need to replace passenger side view mirror 200 GMC Jimmy Diamond

On my '97, there was a triangular shaped piece of plastic on the inside trim that had to be removed. I think all three mounting screws were under that. It's been a few years, but I think that was how it was done.
I remember it was pretty easy though.

Rearview mirror fell off windshield! Not broken...

the mirror should simply slide over the metal mount (button) that is glued to the window.

loosen the set screw on the mirror mount...

hold the rear view mirror mount on the glass just above the metal mount (wide end of plastic opening facing down)

slide it into place...
secure it by tightening down set screw you loosened earlier.
