I agree with Krazy.... I own a 98' D. and generally keep it tuned up every 20-30k. You might also check the air filter as well. Regular service will keep your D on the road a long time.
If you have the cash, get a K&N filter for it. It will help improve your mileage slightly, and a little added acceleration never hurts. :)
Something fun:
I stage 2 chipped mine, put the K&N cold air intake kit (replaced the air box for the direct air tube), put stainless headers on, replaced the exhaust with 3" stainless high flow. I also replaced the coil, cap, and wires to MSD. The truck will still like the standard Champion plugs. All of this put my little 318 at 17MPG and over 350HP (365ish).

It sounds like you need a new fuel filter. if that is to dirty, it wont allow the correct amount of full to be pushed to the engine. try changing that or using a Fuel treatment in the meantime. Hope this helps :)

It could be any sensor or leak anywhere in the emission system.
The best thing to do is to have a code scanner hooked up to the car to pin point the actual problem. If there ia an autozone or advanced auto near you go there, and tell them you need a code retrieved, if not call different auto part stores to find out if they do that. Takes about 30 seconds to get the code.

YOUR EVAP CANISTER IS LOCATED NEAR THE GAS TANK.IT A SMALL PLASTIC SQUARE BOX WITH SMALL RUBBER HOSES CONNECTED TO IT.
