Question about 2003 Dodge Grand Caravan AWD

2001 Chevy Venture 92K miles - code P0134

P0134 HO2S Circuit Insufficient Activity Sensor 1. I clear the code with my reader but for the past month it keeps coming back. I already replaced the O2 sensor on the back exhaust manifold so I guess this is another one?
This code showed up after we had new spark plugs installed. We had them done by a mechanic because I couldn't get to the back ones. Is it possible something was damaged/disconnected during the process of rotating the engine to expose the plugs?
My wife also says she feels the car hesitating on the highway. The fuel filter was replaced around 60K and the last time this occurred (last winter) I added fuel line dryer and it solved the problem. I mention in case they may be related. Car is still getting good gas mileage and otherwise engine runs well.

I am interested in your opinion of the cause for the code, the locaton of the sensor (if I need to replace it) and can it wait a couple of months to be fixed?

Dunno bout rotating the engine to put those plugs in, but the O2 sensor is right in the way as your arm is stuffed up behind the engine (plugs have to be done from below at the back) so I wouldnt be shocked if you found a disconnected O2 sensor that was never reconnected, or a damaged wiring loom from rough contact with the mechanic's arm. Those plugs are very hard to change so go easy on the guy!

How do i fix a code p0134 for my 1999 infintiy i30

code p 0134 refers to heated oxygen sensor(HO2S) 1 bank 1--no activity detected ===causes wiring open circuit ---heating inoperative ---HO2S Sensor
also refers to oxygen sensor (O2S) --no activity detected ===causes --- wiring---O2S
the HO2S is in the exhaust manifold before the cat converter
the O2S is after the cat converter
check the wires and connections , then replace either or both sensors

P1884 code 2005suzuki forenza P0420 P0134 P0138

code p 420 refers to cat converter system , bank 1--efficiency below threshold ===causes ---cat converter faulty---wiring --heated oxygen sensor (HO2S), 2 faulty
code p 0134 refers to HO2S 1 bank 1-no activity detected ===causes --wiring open circuit -- heating inoperative --- HO2S
also refers to oxygen sensor ( O2S) 1 bank 1--no activity detected == causes wiring ---O2S
code p 0138 refers to HO2S 2 bank 1--high voltage === causes ---wiring short to positive ---HO2S--- ECM
also refers to O2S 2 bank 1 high voltage === wiring short to positive O2S -- ECM
I do not have a listing for code p 1338 but I suspect that it will refer to the other codes in some way
first fix is to check the cat converter is operational ( not cracked and blocking the exhaust system)
next replace the HO2S sensor ( one before the cat converter ) and then the O2 sensor (the one after the cat converter)
all the codes will relate to problems in that area

Possible Causes:

Leaks present in the exhaust manifold or exhaust pipes

HO2S signal wire and ground wire crossed in connector (voltage jumps)

HO2S element is fuel contaminated or has failed

ECM has failed

Trouble Code: P0172

Fuel System Too Rich (Cylinder Bank 1)

Possible Causes:

Camshaft timing is incorrect, or the engine has an oil overfill condition

EVAP vapor recovery system failure (may be pulling vacuum)

Fuel pressure regulator is damaged or leaking

HO2S element is contaminated with alcohol or water

MAF or MAP sensor values are incorrect or out-of-range

One of more fuel injectors is leaking

Trouble Code: P1128

Long Term Fuel Trim Add. Air. Bank 1 System Too Lean

Possible Causes:

Fuel pressure is too low or fuel quantity supplied is too low

Fuel filter faulty

Transfer fuel pump has failed

Fuel injector is faulty (sticking or not opening)

Engine speed (RPM) sensor is faulty

MAF sensor circuit open

MAF sensor circuit shorted to ground

Air leak in the manifold

Secondary air injection system combi-valve stuck open

Secondary air injection system electrical short

ECM has failed
