did you replace ignition madule? I see you chg. everything else but that. they can be fine when cold, but when they get hot quit working.

P0161 02 Sensor Heater Circuit Malfunction (Bank 2 Sensor 2)
check the wiring to the bank 2 sensor, that is the one behind the catalytic converter on the opposite side from the bank where #1 cylinder is located, see firing order diagram below.

Been working on my dad's 97 Caravan longer then i can remember for things you mentioned and have been not able to get him to sell it.
I found that the electrical connections are prone to corrosion and have replaced lots of the wire and pin connection in the wire harness, thank god i have time on my hands.
The instrument panel where the wire harness connects, i found that the solder goes bad and i had to re solder the connector on the back side of the circuit board, where the harness goes so you need to remove the instrument panel from the dash,

if the code said it was running rich then your fuel injectors could be dripping fuel into one of the cylinders. try cleaning your injectors. if that dont work then it could be one of your EGR valves, check for that. and check for fuel filter. One of those has to be the problem. most likely its one of your EGR valves. check it out!
