No circulation is due to one a stuck thermostat or two gas bubbles in the cooling system from improper refilling or a leaking head gasket on the engine.

Should the engine be running when adding coolant to my 2003 cadillac cts

If you just drained it & are refilling, then yes,the engine
should run & warm up until the thermostat opens & you
keep adding.

If it isn't hot & circulating thru the entire system, you won't
get it close to full with the rad cap off

Sounds like your thermostat is not opening. When you replace it (engine cold) make sure you obtain the correct temperature setting (170-190 for example) for you specific vehicle.

Changed thermostat and added coolant but 1997

could be waterpump?
i was going to say rad fan,but if so heater would be hot.
lack of coolant will make heater core to blow cold,if engine heats up with full coolant, usually means poor or no circulation.
water pump?
run engine with full coolant,at operating temp.with rad cap off and look in rad cap hole and see if there is movement of water inside rad.
[rad fan will not come on if water is not circulating through rad water pump?]

Try to replace termostat, Check fancover location and fix if it is needed. A bad fan cover let air circulation without to pass across radiator. Radiator itself must be ok (remember to service it once a year at least) due no turn hot in any other situation. check radiator cap
