Ideals ruff thouhgt it was bad gas. no. plugs and wires ok, starts hard and have to rev it up to keep it going. when it gets warmer it will idile, but very low rpm's.

now my answer based on no engine told
HL are 2nd problem. if battery is good and alternator good
and battery reads 13.3vdc to 15vdc at idle if not ALT is bad.
but you'd know that driving 1 hour or more, it would stall. (maybe)
first off the glow plugs must all be good or cold starter will fail or be horrible in some way.
2nd great fact is runs great engine hot. see that is important too know.
bad wring for HL, the starter motor will fill if batteries are weak
fail hard. that.
so you have power it cranks and if HL fail HL are bad, wiring?
got DRL down under ? daylight running lamps. hope not they stink..
or the main fuse box under hood (bay) the box has rusted fuse terminals, seen many here, 1990 cars. rusted bad.
CALLED BAD WIRING.
this bad in this spot effects all 12vdc power to all things in car.
the fuses are huge 50amps or 100amp or more.
so the answer is get the trucks serviced
a real shop that knows electrics and diesels both.
tell them what said, and they will find it, even just looking .

and a glow plug test. for sure.

you may have bad or very dirty fuel injectors causing too much fuel into the cylinders causing rough running and smell of gas. do you have a fuel leak or too high fuel pressure.

CHANGE SPARK PLUGS AND SPRAY WATER WITH A SPRAY BOTTLE LIGHTLY OVER PLUG LEADS AT NIGHT AND REV ENGINE IN PITCH DARK ..........IF U SEE BLUE SPARKS TRACKING ALONG OR AROUND PLUG WIRES YOU KNOW LEADS NEED REPLACING ............OR CHECK TIMING HAS NOT JUMPED ONE TOOTH

take that stinking van back and replace it with a ride you like and tell them what it was doing because that is crazy

I had the same problem with my 93 K2500 it only has 159000 on it. It was my catalytic converter, drop the Y pipe going into your exhaust manifold then drive it. You should feel an immediate difference. You can replace it or cut it open if your state doesn't have emission laws. It cost me around 250 and a couple hours of work.

The throttle body may be partially blocked.
Clean the throttle body

Try having your fuel injector cleaned, you can start with running fuel injector cleaner though your gas tank and if that does not help then you have to get it cleaned out professionally or even worst have the injector's replaced if they are bad.
The other thing that will give it bad fuel would be a bad air intake temperature sensor go bad, as for the ruff idle that is common to a dirty or stuck EGR valve.

might be the coil pack is bad, wires, plugs, o2 sensors, dirty injectors, catalic converter ( only if you smell rotten eggs ) . cylinders are misfireing.

dont go changing bits ,then find it still doesnt work ??chances are they are right but take it to garage as an incorrect diagnosis will cost you money in the long run

did you have your fuel pump looked at! mine did the same thing ,had to replace tank, cause of small rust holes and fuel pump was bad. this cost ed me 850.00 to have fixed. hope you have better luck!
