Sorry this is not an easy fix the clutch packs in the transmission are slipping, usually caused by wear, but the hydraulic pump is probably getting week. As the oil warms up the oil gets thinner and the hydraulic pressure decreases. Times for a overhaul of the transmission, and if the torque converter can not be flushed a new torque converter is recommended. You may get lucky buy changing the fluid several times, but if the transmission is 22 years old it may not help.

Try Seafoam available at your local auto parts store. You either have a moisture build up in your fuel system, or a carbon build up in your exhaust system. The Seafoam additive will get the moisture and carbon out yu may also try higher octane gas 89 or 93 for a tank or 2. This is a common problem with the 5.9 and 5.2 V-8 engine go to youtube type Seafoam for video
