Hi Daniel, The problem sounds to me as if it has to do with the temperature sensor which sends signals to the ECU. The sensor is nothing to do with the gage on the instrument panel, but instead signals the the ECU to increase fuel richness for cold start and cold running. Have a diagnostic scan carried out, and check for disconnections or broken cables, if the connections and cables are good the defect may not show on the scan, but replace the sensor anyway. I actually had the 02 sensors changed (even though my check engine light never came on) and the IAC (Idle control Valve) and it seems to be running fine.

Sounds like the fuel filter is dirty and needing to be changed. It's cheap and easy to swap out.

To update my earlier repair on the 2001 6 cyl Escape: When cold, and when the ignition was turned over but no accelerator pressure given, the rough idle finally returned the P0303 code again when the check engine light came on indicating that it wasn't caused by the plugs or the coil. FYI, I had replaced the intake manifold gaskets during the original repair and just prior to that I had replaced the fuel filter which was very dirty (I cut it open to see).
The repair manual referred to fuel injectors as a possible problem. I have run three tanks of fuel using different injector cleaning products and am very pleased with the results. Cold starts are much smoother and I am unable to coax out the P0303 code or any other code.
My opinion, change the fuel filter, use a fuel injector cleaner for a couple of tanks, change the plugs if the problem still exists. The coils were not the problem on my vehicle.

fluctuates at idle up and down or dies near idle.
This problem is commonly caused by a dirty automatic idle speed control valve and throttle valve but always run a OBD2 fault code as well as the cleaning procedure. Buy a can of throttle valve cleaner (do not use carburetor spray cleaner!) from NAPA or Carquest (made by CRC chemicals) and spray it into the air intake while the engine is running, use up about 1/2 the can, engine will try to stall hold the speed up, shut it down and let it soak for 30 minutes, restart and blow out the remaining fluid, shut it down and disconnect the negative battery cable for 5 Min's to reset the base idle control. If the above cleaning doesn't cure your issues you need to have the computer controlled automatic idle speed control system diagnosed and repaired by a dealer or electronic engine control repair shop.
