1. where is the transmission control module located..............
2. and where is the shift solenoid located....................

The most likely cause of this is either the transmission control module or a defective shift solenoid (in the trans itself), this is an electronically controlled transmission. Early 90's Honda transmissions are prone to this issue, the control module is the most common cause.
