Likely, the "overdrive lockup" is malfunctioning. This is internal and requires that trans be removed for service. Have an honest, competent transmission shop evaluate your problem. Transmissions are expensive, so unless the remainder of your vehicle is in really excellent shape, the cost of a rebuild may not be a valid expense (only you know that) consider buying another vehicle. Transmissions are no longer a quick easy backyard repair!

Is it better to go into D3 wihile car is moving or stopped?

Well every gear has its limitations. I am not sure what the mph ranges for each gear are, but i've gotten to 60+ mph in 3rd gear. The only thing about driving on the freeway in 3rd gear is that it is alot less efficient mileage wise. Your turning more RPMS while on the freeway in 3rd gear so you burn up alot more gas.
Driving in D3 is driving in 3rd gear. It could burn up the tranny but it depends on the vehicle itself. It is not really made to do highways speeds in 3rd gear. It should be in 4th or 5th (OD)

GM uses a "ignition module" before installed there is a paste to disapate heat that is smeared on the back of the module. after time the paste thins and dont protect the module from heat. my sujestion is to replace the module it is around 80-100 dollars ( BE SURE TO SMEAR THE PASTE COMPLETELY OVER THE BACK OF THE MODULE ).
