I have an early 2004 dodge ram 2500 w/ 5.9. every time i start the truck the volt meter shows about 11.5 volts. after approx. 5 min. of run time the volt meter shoots up to 14+ volts. increasing rpm's does not change the output reading. is my alternator on it's way out?

when i try to shift into four wheel drive it doesnt always go. it seems to get worse when its cold outside. i think it has something to do with the vacuume lines or the vacuume lock on the front axle. it is a manual transmition and the four wheel drive dash light does not come on even when it is in four wheel drive.
