The 4th gear clutch shaft has very fine teeth that wear over time, if vehicle has high miles, this shaft may have failed, a sprag may have failed, the 4th gear selenoid may be bad, the 4th gear clutches could be fried, the valve body may be leaking / venting clutch apply pressure, the range selector may be off causing the VCM to believe you are in 3rd. Good luck!
