Driver side headlamp is out and bulb is still good, dealership says it is the headlight control module, so where does it go?
Very simple remove the front radiator sheild it should have 6 plastic clips on it. if you dont know wat that is its the big black plastic cover right when u pop the hood. u will see a 10mm bolt on the top of the headlamp assembly remove that bolt you will also see a screw on the other side on top of the parking lamp assembly remove the srew after the screw is removed u will b able to slide out the parking assembly after that has been removed u will see one more 10mm bolt holding the headlight assembly in place remove that and the headlight assembly will slide out after it is out unplug the 3 electrical connectors from the back of the assembly remove the assembly. if u flip it over their will be four screws on the bottom of the assembly remove the four screw and take the plastic plate off u will see a steal box that is ur headlight CM remove the two electrical connectors off the box one of them goes to the headlight twist off the headlight waterproof cap and un plug.. should take you no more than 30mins

no to both questions. we have had good luck with wet sanding the plastic faces with 1500 sand paper than with polishing compound buff them back to new.

you have to remove the head lamp. there are 4 bolts that hold it in. you will have to remove ouyer marker lens first.1 screw on top and pull forward. clips hold lens pretty tight.

There is a clip that pries up with small flathead screwdriver. The harness and bulb are held in lens with a plastic ring that screws on and off.

This is one of the easiest fixes you can do on your lexus...all thats needed is just unscrew the canisters' holding the lightbulbs, they look like dome fixtures encasing the HID lights...once you unscrew those, the bulbs are held in place by simple snap components that actually hold the lights in place...pull the old lamps out...unplug them, and then reverse this procedure....it took me all of 15 minutes, and a savings of $1000 to do this...
