Cylinder # 3 has a misfire...#1) Defective coil on plug or the spark plug itself...#2) Contaminated fuel injector...See drawings below...The coil pack is the smaller of the two and sits to the rear of engine it is the most probable cause...Although the spark plug or the ignition wire (connection between plug and coil pack) could be at fault...so, before you invest in a new coil pack...Remove from both ends the wire from #3 and switch it with #2...if the fault code changes to p0302 that wire is bad...next Install a new set of plugs....Fault still present? replace the coil pack.....Unless recently changed new plugs and wires are a plus even if fault remains...saailer
Cold starting misfires can often be caused by either tired high tension spark plug leads or ignition coil packs. If replacing leads, then replace with a full, new set. Coils can be changed individually, but it is often the case that if one has failed, then the others aren't far behind. Some cars have individual coil packs on each spark plug. Some cars have a single pack for all plugs and some have coil packs which may control two or more plugs.
To check for a misfire (ignition failure) follow these steps:
(Cold start) (Use a pair of insulated pliers)
1) Start the vehicle
2) When the vehicle starts misfiring, disconnect plug leads or coil pack connections one at a time. If the vehicle idles even more roughly, then that plug or pack should be considered fine.
Replug each connection before disconnecting the next one. Once you disconnect a plug lead or coil pack connection and find no difference in poor engine running, then you have likely found the offending cylinder. Check the spark plug condition, lead and/or coil condition and connections on that cylinder.

the PCM monitors the rail coil , the 3.8 has a 3 COIL pack.

so what is the first thing to do here.
easy , put in new spark plugs.
try doing the tuneups. in the glove box guide.

mine has only 60,000 miles, and they both were bad.
spark plugs and HV, wires so I upgraded to NGK mag wires.
and new NGK plugs.
if it still fails, then try a new coil pack.
Dont get another harness. If you are using Autozone supplied coil packs, the problem is most likely the coils from Autozone. I have went through this exact same thing before. I changed the coild because one had a burn mark on the boot between the plug and the coil pack. I noticed it when changing spark plugs. The new coil from Autozone made the vehicle struggle on take offs and idle rough and the vehicle had no power. I installed another coil from Autozone again which was supposedly a better model and costed more. The vehicle ran a little smoother but the rough idle and small misfire was still there. I installed another coil from the dealership and the problem was fixed immediately. I also replaced one of the coil connectors for a different cylinder but only because i broke it when disconnecting the coil packs to remove the spark plugs. And when I installed all my coils and the new one from Autozone, I used dielectric grease on all the connectors. But the rough idle and power loss was immediate following installing the new coil from Autozone. Beware if you are using coils from another part store because many part stores get their parts from the same source.
- Remove any plastic covers over the engine - Locate the spark plug wires, and where they run to at the engine head. - If your vehicle has a distributor, simply pull the plug wires off of the spark plug. If the vehicle has coil packs, you will have to pull the wire out of the coil pack (by releasing the connector) and then remove the coil packs. - Use a spark plug socket with a ratchet and 6" extension to loosen and remove the spark plugs. - Gap the new spark plugs to the proper specification, and then assemble everything in reverse order that you removed it.
1. Check the gap at the base of the new spark plugs with a spark plug gap tool. If the gap on any plug is not between 0.039 and 0.043, exchange the plug for a new one. Do not attempt to adjust the gap on the plugs. 2. Open the two latches on the passenger's side of the air-filter box. Pull the front half of the air box toward the front of the engine and push it to the side. Remove the air filter from inside the air-filter box. This allows you access to the ignition coil packs. 3. Unplug the wiring harness from the top of one coil pack. Remove the bolt securing the coil pack with a ratchet and socket, and pull the coil up from the engine. 4. Remove the spark plug using a ratchet, 6-inch extension and a spark plug socket. 5. Hand-tighten the new spark plug into the RAV4's engine and torque it to 14 foot-pounds with a torque wrench, 6-inch extension and a spark plug socket. 6. Squeeze a small amount of dielectric grease into the rubber boot at the bottom of the ignition coil pack. Push the coil pack onto the new spark plug until it clicks into place. Tighten the coil pack bolt to 6 to 7 foot-pounds with a torque wrench, 6-inch extension and a spark plug socket. Reconnect the wiring harness on the top of the coil pack. 7. Replace all four spark plugs, by repeating Steps 4 through 7 on all four plugs. 8. Sit the air filter back into the air-filter box. Reinstall the front half of the air-filter box and close the latches to lock it in place.
The spark plugs receives power to spark from coil pack.Check the power at the coil pack and also check the voltage at the coil pack with meter.If the coil pack shows proper power as well as voltage then its fault in spark plugs.Otherwise its faulty coil pack.The coil pack receives power from battery.so also check the voltage of the battery it should be exact 12 volt. Have you checked the spark plug its self. You can try to see if it sparks to ground. To do this try to touch it to the engine block while cranking the engine. If nothing get a multimeter reading from where the pack plugs into the spark plug. If theres power there then I would recommend changing the spark plug. I hope this helps please rate this thank you
