You have oil entering the radiator and when combined with water makes a milky substance. Oil entry is normally through a defective cylinder head gasket.

So sorry to hear of this Problem, But You have either A head Gasket that has Acquired a Crack or A crack in the Engine Block somewhere between the Cylinders and an Oil port. this is what will cause Milky Oil in the Coolant reservoir/overflow tank, if the color is More Brown then it is Definitely engine Oil if it is Reddish then it is a trans coolant leak But since You say you replaced the Radiator then My money goes to and Engine Problem, You nee to See a Professional Mechanic shop to have the System Pressure Checked, And The Oil Dipstick checked, if You have Milky foam on that stick then You will have narrowed down the Problem

If the milky substance is in your motor oil. You could have a cracked head, broken gasket, or a cracked block.

What causes radiator water to be milky looking chrysler pacifica 2006

It could be an oil leak in your head gasket. when oil and water mix in your motor the result is a milky grey substance, I have only seen it in the block oil pan, not the coolant resevior.

Antifreeze has a white millky substance to it especially in coolant bottle and in hoses. Mechanic say that radiator is failing. Is this true?

Check the oil and also transmission fluid, one of them should also be milky. If it's the oil that is also milky then most likely cause is blown headgasket. If it's transmission fluid, then the radiator probably has failed in the transmission cooler section. If the transmission fluid is milky, then you also need to have the fluid flushed and the trans. filter changed after fixing the radiator.
