You said you have fuel pressure but what is it? It has to be 55 psi or it won't even try.

SOURCE: rough idle, runs fine on highway

It is possible that the regulator is leaking into the intake plenum, or one of the injector lines has a bad O-ring and is doing the same thing. This was actually a big problem with this system on the 4.3L engines, but it is a little different on the 5.7. Before tearing into this this though, I think it would be a good idea to get a professional diagnosis. I would start with a compression test. The miss could be caused by low compression in one or more cylinders. The injector assembly has the regulator mounted right on the side of it and the whole assembly resides inside the intake plenum. It is not that difficult a job, requires some patience and skill, the worst part is the cost of the parts.
I've attached a picture of the injector assembly with the reg attached and highlighted

1989 GMC Sierra 1500 heater blows cold air. Changed thermostat. Both hoses are cold.

check antifreeze level add if low, and check intake heat control valve on ps side of intake if its locked shut it can be removed and a water pipe nipple put in place of it and the heater hose reconnected..if the water pump is good, it should give heat this valve should be on the intake with a heater hose connected to it..and its vacuum operated but will jam shut with age and cold weather..good luck..

starting definately has a lot to do with the crank/cam position sensors, but it wouldnt have nothing to do with hot or cold weather, oxygen sensor will make the vehicle run too rich if its out, you will smell fuel very strong out the exhaust, that could make it run sluggish also, and usually if the mass air flow sensor goes out the vehicle wont start at all...

Heater blows cold

ok there is a few things might be happening...first did you make sure when you put the intake gaskets in there was No ports plugged or covered...second is...are both heater hoses Cold or is one hot and other cold...if so you have a air lock in the heater core...if this is the problem you need to lossen off the cold one till antifrezze comes out.

2006 GMC Sierra electrical problem

If your Sierra has a Duramax, it may be the intake air heater or the fuel heater cycling on and off in the cold weather. But if its pulling the voltage down to 12V after warmup, I would suspect a poor battery. As suggested, get a load test at autozone or ??
