Brakes are making a grinding sound

If your brakes are grinding I would take the tires off and check the pads and rotors for extreme wear. The grinding sound could be the pads are so wore that the metal pads are digging into the rotors. If this is the case before putting the pads back on replace or cut down the rotors if there not to damaged.

there is a parkingbrake inside the hub of the rotor try hitting the hub with a hammer as hard as you can swing a couple of times without hitting the wheelstuds there may also be clips on at least 2 studs if the rotors havent been off remove these clips dont worry if you break them you wont need them the rotor should come off

How to remove rear rotors?

You need to remove the calipers and then the caliper mounts.Once that has been done then you just need to remove the rotors from the hub,it may take some hammering or even some heat but they will come off if you are persistant enough.

Changed the front brakes on my 2005 Grand Caravan and now there's clicking and serious vibration at speeds above 40mph. Also, I need to bleed the brakes, b/c the brake pedal doesn't respond normally, and I...

sounds like the pads were either installed wrong or your forgot the have the rotors ground down. grinding down the rotors would have taken out any extra vibration. if the rotors arent ground flat, the pads will cause vibration as they seat against the uneven surface of the rotors. the clicking indicates that the brake calipher was not secured and is hitting the wheel when the brakes are applied. PLEASE Get a Manual from chiltons or haynes for your car they will help you change your pads and rotors more efficently.

They should just come off. but may need some hammer to assist. they tend to rust up like most other rotors. nothing special needed.
