Also it would be a good idea to check you evap system. this system controls you evaperative gas emitions from tank. if the control solieniod is stuck or the canister is full of dirt it will no left air out of the tank. so this causes the tank to pressure up when refueling. that can cause this problem

Some 95 E vans were made for diesel ,on request, but that would make the hole opening larger. The regular is large too and the smaller opening is for unleaded gas. This may be a customVan with a screen filter that blocks the insert of improper nozzels. Check the depth of the screen with a tape measure. the depth should be 8-10 inches deep. Someone may have tried to remove the screen to steal gas and failed.and the screen has been pulled upward or damaged. If it is the diameter that is the problem then the opening may have been damaged by unknown sources. ei vandalism. Also the screen may be totaly blocked in the neck of the opening. Try inserting a rod through the screen, (with a small metal rod) and then pour a small amount of gas to see if it filters in. If it does you may have to remove the screen completely.
