Periodically the pickup will die when coming to a stop (i.e. pushing in the clutch and allowing the engine to drop to idle without a load) It seems to idle fine, and accelerate OK. The engine has about 10K miles on a full rebuild. Chassis has 230k. As far as I know the injectors are original. New air filter and all the electronic 'doo dads' seem to be plugged in solidly and making a good connection. There is a vaccuum (I assume) screw on the engine side of the throttle 'butterfly' (I would call it a butterfly on a carburator) that my son fiddled with, assuming that was the idle control. It seats solid, and I've backed it out two full turns. Don't have the book. Help? :)
I would clean the idle speed control valve (IAC) and the throttle valve and bore with CRC brand throttle valve cleaner, use about 1/2 of the can, hold the idle up so it won't die while you spray it into the throttle valve. After you are done disconnect the battery negative (-) cable for 5 min's and then drive it for a few days so the system can relearn the idle strategy for your driving habits
In addition to the above I suggest disconnecting the battery overnight and then driving constantly in a mixed fashion for at least 20 minutes.
If that shows no improvement it might need some dedicated diagnostic equipment to discover the problem. 230k is a long way for fuel injectors and it could be the spray patterns aren't very good and that is just the sort of thing to cause questionable running as you describe.
I strongly suggest you try a dedicated Toyota forum to discover the purpose of your mystery vacuum screw and the correct method of adjustment.

HAVE YOUR EXHAUST CONVERTER CHECKED! IF IT'S BAD AND YOU HAVE IT REPLACED, DON'T LET THE ONE THAT REPLACE'S IT TAKE YOUR OLD ONE!!!!! IT'S WORTH ABOUT $250.00

your throttle position sensor might me bad or out of adjustment. if it is out of adjustment then it can misfire all the way up to 3500 rpm.
i had a friend that had a miss fire at 2500 rpm. i adjusted the tpss right and the misfire went away. Check for engine codes by jumping TE1 and E1.
Your fuel filter may be dirty and restricting fuel to your injectors.
have your fuel pressure checked.
SOURCE: 1991 Toyota pickup 4WD clutch slipping
There is an adjustment for the clutch pedal under the dash where the push rod goes thru the firewall. There is only aprx. 1/2" of adjustment so be careful when making an adjustment, if your clutch is slipping then you need to shorten the throw of the push rod.

check all vacuum hoses.if any looks cracked replace them.make sure carburater bolts are tight.check the egr valve make sure its not leaking.besure have your idle speed adjusted.it should be at 550 to 600 rpms.

Did you connect the lines? and did you break loose the tar that holds itto the car and seals the holes for the pipe? You maya have to remove the outter core cover to getto the last screws to get it out.
Engine stalls cold or RPM
fluctuates at idle up and down or dies near idle.
This problem is commonly caused by a dirty automatic idle speed control valve and throttle valve but always run a OBD2 fault code as well as the cleaning procedure. Buy a can of throttle valve cleaner (do not use carburetor spray cleaner!) from NAPA or Carquest (made by CRC chemicals) and spray it into the air intake while the engine is running, use up about 1/2 the can, engine will try to stall hold the speed up, shut it down and let it soak for 30 minutes, restart and blow out the remaining fluid, shut it down and disconnect the negative battery cable for 5 Min's to reset the base idle control
