Its a hose at the front of where the passengers feet area under the hood. Usually the problem is water holds and leaks on the floor. A repair shop may have a spray that they can coat the evaporator and kill the smell.
The mildew smell you refer to could be anti-freeze. When a heater core starts leaking it may allow anti-freeze (which circulates through it for heat) to be blown through the ducts onto the windshield in defrost mode. The repair is to have the heater core replaced. It may wait for the thermostat to open and allow the coolant to circulate before you notice it. Occasionally the carpet on the passenger side may be wet from leakage.
Look under the passenger side of the dash, behind the carpet. Feel the back side of the carpet for dampness. If so, try this: In the same area, look for a black rubber hose that comes from the heater plastic box, and pokes thru the firewall. See if it is plugged up. This is the drain pipe for the AC box, and the cowl panel if the side drains are plugged up. Try that, and please get back to me.
